For over 50 years, CCHN has been committed to providing exceptional medical services to diverse communities, including the underprivileged and marginalized. Our collaboration aims to amplify this mission by offering a digital platform that is as inclusive as it is innovative.

The website is a testament to CCHN's dedication to accessible, high-quality healthcare for all, regardless of demographics. It serves as a Patient-Centered Medical Home, offering an array of services from general healthcare to specialized treatments. The site is designed to be multi-linguistic, ensuring that language is not a barrier to essential healthcare information. This feature is particularly impactful in communities where English may not be the first language, thus empowering more people to take control of their health.

One of the standout features of the project is its focus on community care. CCHN helps over 60,000 people annually, many of whom are from underprivileged backgrounds. The website reflects this commitment by offering a low-cost pharmacy program, health education classes, and on-site lab services. It accepts most insurance plans, including Medicaid and Medicare, and even offers a Sliding Fee Discount Program for those who qualify, making healthcare affordable for all.
